Community Annual Planning Submission
The Local Health System Integration Act 2006 (LHISA) requires that the Hamilton Niagara Haldimand Brant Local Health Integration Network (HNHB LHIN) sign a Multi-Sector Service Accountability Agreement (M-SAA) with each health service provider that it funds. The Service Agreement for Community Services assigned by the Minister of Health and Long-Term Care to the LHINs, on April 1, 2007, must be replaced by April 1, 2009. Accordingly, it is the HNHB LHIN’s intent to have a signed M-SAA in place with its funded community health sector agencies; community health centres, Hamilton Niagara Haldimand Brant Community Care Access Centre (HNHB CCAC), mental health and addiction and community support services by March 31, 2009.
The 2009-11 Community Annual Planning Submission (CAPS) is now available on the Web Enabled Reporting System (WERS). Community mental health and addictions, community support services, community health centres, and the HNHB CCAC are required to complete the CAPS and submit it on the WERS by November 30, 2008.
