Health System Improvement Pre-Proposals (H-SIP)
The HNHB LHIN has responsibility for planning, funding and integrating health services at the local level. The LHIN works in collaboration with health service providers (HSPs) and related stakeholders to ensure the development of a responsive and sustainable local health system that provides quality care and improves the health status of the population.
The Triple Aim (Institute for Healthcare Improvement, 2008) framework guides the LHIN’s approach to health system improvement. The three intentional goals of Triple Aim are as follows:
- to improve the health of a defined population;
- to enhance the patient care experience; and
- to reduce or control the per capita costs of care.
All three aspects are equally important and ideally are addressed together. In today’s economic climate there is an increasing imperative to provide healthcare services that are not only of the highest quality but are also effective and efficient.
The H-SIP pre-proposal is designed to capture the intent and scope of ideas for health system improvement. A proposal should demonstrate how it will improve population health, improve the patient care experience, and contain or reduce health system costs.

LHIN Urgent Priorities Funding
The Ministry of Health and Long-Term Care made resources available to help us address the priorities set out in our Integrated Health Service Plan as well as emerging pressure points. This LHIN Urgent Priorities Fund (LUPF) totalled $2.9 million for 2007-2008 and $5.6 million for 2008-2009.
Over the course of six months, (October 2007 to March 2008) health service providers submitted proposals to the HNHB LHIN which were assessed against three principle criteria: ‘Is the proposed initiative of benefit to the community?’ ‘What are the risks involved?’ and ‘How feasible is the proposed initiative?’. Proposals were reviewed to ensure they advanced our priorities or emerging pressure points.
LUPF is now finished for the 2009/2010 fiscal year. If the Ministry makes more funds available in the future for LHIN Urgent Priorities, we will repost the forms and information. Aging at Home Funding
The Aging at Home (Aging At Home) Strategy was announced by the Ministry of Health and Long-Term Care (MOHLTC) in August, 2007. The goal of the strategy is to enhance healthy aging and independent living for people at home and in their communities as they as get older. Approximately $60 million will be invested in the Hamilton Niagara Haldimand Brant Local Health Integration Network (HNHB LHIN) over 3 years.
We have recently allocated more than seven million dollars to initiatives designed to support people at home in their local communities as they get older. The projects rolling out in year one are only the beginning; and a very exciting beginning.
