Integrated Health Service Plan 2010-2013
At its December 15 meeting, the Hamilton Niagara Haldimand Brant Local Health Integration Network (HNHB LHIN) Board of Directors approved its second Integrated Health Service Plan (IHSP) entitled Improving our Health Care Experience: Integrated Health Service Plan 2010-2013
This second IHSP is a road map for health system change for the next three years, 2010 - 2013. This IHSP is informed by Ministry of Health and Long-Term Care priorities for Ontario, and local priorities. It will guide the HNHB LHIN’s annual business plans, integration priorities, and funding decisions for the local health system.
The Integrated Health Service Plan covers the period April 1, 2010 to March 31, 2013 and focuses on the following priorities (grouped by colour as they are presented in the IHSP). Working Groups
A number of working groups will be established to carry out early actions from the IHSP. As part of a patient flow priority, these working groups will work on developing strategies for LHIN-wide coordination of clinical programs.
The Clinical Services Plan (CSP) was a significant building block for the IHSP. The CSP's key directions for health system improvement - interprofessional care, clinical program integration and community-based health service capacity, are incorporated into the IHSP. Further Reading: In 2006, the HNHB LHIN released its first Integrated Health Service Plan (IHSP). The Plan guided early priorities for health improvement and funding decisions over three years.
