New CDPM Lead for HNHB LHIN 

The HNHB LHIN would like to thank Dr. Akbar Panju and Dr. Nasima Mottiar, who have recently completed their two year term as Chronic Disease Prevention and Management (CDPM) Co-Leads for the HNHB LHIN.

Their contribution as champions for CDPM in our LHIN is exemplified in their many achievements which include: two Continuing Health Sciences Education Events, completion of the Diabetes Action Group Report, identification of a risk classification and management approach for diabetic foot care and expansion of diabetic foot care resources across the LHIN.

Going forward the LHIN will coordinate CDPM activities with the implementation of the LHIN’s Clinical Services Plan and Integrated Health Service Plan.  The LHIN is pleased to announce that Dr. Jennifer Everson HNHB LHIN Physician Lead for Clinical Planning and Integration will lead the LHIN’s CDPM portfolio.


HNHB LHIN Hosts Second Raising the Bar in Chronic Disease Prevention and Management Continuing Health Science Education Event

A wide mix of health care professionals from across the HNHB LHIN attended the second Raising the Bar in Chronic Disease Prevention and Management Continuing Health Science Education Event on November 4.  This was an opportunity to increase the uptake of best practice guidelines and was a part of the LHIN’s chronic disease prevention and management strategy.  The event was accredited through McMaster University’s Continuing Health Sciences Education office.

Last year, the HNHB LHIN surveyed health care professionals who serve individuals with diabetes. The purpose was to help the Diabetes Action Group (DAG) identify initial strategies to improve diabetes management and outcomes for people with diabetes. Click here for an overview of the survey results.

Collaboration Works!  Members of the Diabetes Action Group Complete an Inventory and Categorization of Diabetes Education Materials for Health Service Providers 

The Diabetes Action Group (DAG) of the HNHB LHIN formed a subcommittee to complete the inventory and categorization of diabetes education materials.  This document is the addendum to the December 2008 DAG Report and presents the processes, results and findings of the inventory and categorization of diabetes education materials used by HNHB LHIN diabetes providers and stakeholders. 

Continuing Medical Education on Chronic Disease Prevention and Management

On January 7, 2009 the Hamilton Niagara Haldimand Brant Local Health Integration Network held it’s first Continuing Medical Education (CME) Event entitled Raising the Bar on Chronic Disease Prevention and Management (CDPM).  The event was attended by over 100 participants!  The participants were physicians, registered nurses, dieticians, nurse practitioners, health care administrators and other health care professionals.

The CME, which was part of a larger Chronic Disease Prevention and Management initiative, was integral in raising awareness that all health care providers can play a part in CDPM.  The feedback from the event was extremely positive.  The formal evaluations solidify the fact that the event was highly informative, thought provoking and inspiring.  The mix of professionals in attendance at the CME showed that health and wellness experts from across the LHIN and health care sectors are committed to improving chronic disease prevention and management

The event was accredited through McMaster University and participants earned the College of Family Physicians MAINPRO-M1 and The Royal College of Physicians and Surgeons of Canada learning credits.  According to participant evaluations almost 90% of the respondents agreed or strongly agreed that the CME met the outlined learning objectives.

The format consisted of four expert speakers who also participated in a panel discussion along with an additional expert panelist.  In the words of participants they found the panel discussion to be; Very Informative, very worthwhile to hear others’ view points and it was good to bring everything together to move forward.

Additional comments from participants included the following:

  • “Very well done, lots of input into the future of better managing chronic disease.”
  • “Extremely well organized.  Excellent planning to organize such a successful event.  Thank you!”
  • “Well balanced and informative.”
