The Hamilton Niagara Haldimand Brant LHIN delivers and coordinates home and community care services. If you, or someone you care about, needs health care services at home, at school or in the community, or if you are considering supported living programs or long-term care options, home and community care at the LHIN can help.

The LHIN works with people of all ages to ensure they can make informed choices about their care, when and where they need it.

We also have useful information about local community support service agencies, and can link people to these providers to arrange services.

Recent Changes Related to Home and Community Care

On May 10, 2017, home and community care services and staff transferred from the HNHB Community Care Access Centre (CCAC) to the Hamilton Niagara Haldimand Brant LHIN. Home and community care services are now provided through the LHIN. The LHIN plans, funds and integrates local health care and also delivers and coordinates home and community care.

As part of the Government of Ontario’s Patients First: Action Plan for Health Care, this health system change was seamless in order to ensure uninterrupted care and support for patients and home care clients.
