At its June 29, 2010 meeting the HNHB LHIN Board of Directors approved a plan to create an LHIN-wide integrated program for complex care. The plan includes seven recommendations.

On June 29, 2010, the HNHB LHIN Board of Directors approved the Annual Business Plan 2010-2011.  The Annual Business Plan summarizes our planned activities to discharge the LHIN mandate to achieve transformation and to maintain accountability to the stakeholders of our community.

This Health Atlas from the HNHB LHIN identifies and outlines a broad set of health indictors.  The purpose is to provide context for planning, monitoring and evaluating health system performance within the context of local population health needs.

This second Integrated Health Service Plan (IHSP)  is a road map for health system change for the next three years, 2010 - 2013. This IHSP is informed by Ministry of Health and Long-Term Care priorities for Ontario, and local priorities. It will guide the HNHB LHIN’s annual business plans, integration priorities, and funding decisions for the local health system.

Nov 24, 2009Clinical Services Plan Final Report
In January 2009, the HNHB LHIN launched the development of the Plan. The Plan was one of a series of initiatives to identify requirements for an accessible, effective and sustainable health system. The outcome is a three-plus year strategy map to reduce illness, improve health outcomes, and transform people’s experience in the health system.
